In order for RMC to officially change your name, we require that you provide the following documents:
1) Updated Driver's License (via email to Payroll@RMCEvents.com) 2) Updated Social Security Card (via email to Payroll@RMCEvents.com) 3) Updated Tax Forms (W4 and VA-4, both can be found on our website)
In order for DCJS to process your name change, they require ONE of the following documents:
1) Marriage Certificate, license, or register 2) Divorce decree 3) Court order granting the name change
You will need to bring one of these documents along with a check for $34 made payable to "RMC Events" to your nearest RMC Office. RMC will then deliver the necessary documentation to the DCJS office for processing.
